After so many tries i finally achieve to unblock hell world. The question is, can i play it in all difficulty modes or do i have to remain in this doomed nightmare ?Thanks

Congratulations! 
After you get the message about unlocking Hell World, start a new game instead of closing (shutting down) the game. You'll have Hell World unlocked, rewards from completing Nightmare, and you'll be able to choose the mode of difficulty.
