Once you get Yveen, enter the building. In this first room, you can immediately (1) take a left and go up, (2) take a right and go up, or (3), go straight up. Go straight up to the room with a pedestal and a chest (contains a strange armor, Czork-Gmogr). To the upper left and upper right of this pedestal are 2 disc-orb shaped structures. Enter the disc to the right to find SR 191. The entrance to SR 191 was missing for a long time and is only present in the latest build, sometime earlier this year. If you don't have that latest build, you won't be able to enter SR 191. There's a nice spear there, but it certainly isn't necessary to win the game.
