I got the Nirium Set, do I have to go back to the Fairy or Witch (don't remember)
to let her know that I got the Set?
Where is she?
The 4 items for the Witch, by mistake I use the Rafelesia berries, I have the others.
What should I do?
I want to tied those loose ends to go after the Dragons.

I don't think the fairy who told you about the Nirum set will ever show up again.

As for your second question... I don't know if there's any way for you to get more berries at this point, so I'll leave this question to someone else. :P
